I think there are quite a few places you can find online that will do custom bikes. I imagine alot of them aren't online because they the internet tends to be a place that bargains are searched for, and a custom bespoke bike setup is not something that the builder is going to offer on the cheap. That being said, it is not too difficult to get frameset and even full bikes built to custom specifications. It kind of depends on what you consider a bespoke bike. Do you mean every inch of the frame custom to your body? Do you want a builder to build a bike to your specifications or somewhere in between. A couple that I can think of offhand are http://www.tjcycles.co.uk/ makers of the Flying Gate frame. The have couple different frame options that can be built to your specifications. I do not know if they can measure you etc but they make every frame custom for whomever orders it - and the flying gate is insane. The down side is I dont think they build up the bike after the frame. Another option is http://www.fixedgearlondon.com, from what I know they just assemble bikes to custom specs but dont build frames. And another would be http://www.freshtripe.co.uk, I think they get their frames built offsite and then build up the bike. Regardless these options are all one way or another bespoke bicycle.
